Abstract

Chronic stroke is a leading cause of long-term disability, potentially resulting in total dependence for activities of daily living. This condition necessitates ongoing assistance for basic needs and places patients at risk of various complications. Palliative care is an approach focused on enhancing the quality of life for patients and their families by addressing physical, psychological, social, and spiritual needs. This case study aims to describe the application of palliative nursing care in meeting the basic needs of a patient with chronic stroke and total dependence. A case study method utilizing the nursing process comprising assessment, diagnosis, intervention, implementation, and evaluation was employed. The study focused on Mr. S, a 69. year old patient diagnosed with hemorrhagic stroke and total dependence. Assessment findings identified nursing issues including impaired physical mobility, self care deficits, ineffective airway clearance, risk for aspiration, and risk for impaired skin integrity. Nursing interventions focused on addressing needs related to oxygenation, nutrition, personal hygiene, mobilization and complication prevention, alongside providing psychosocial and spiritual support. Evaluation results demonstrated improved patient comfort and airway clearance, the successful meeting of basic needs, and the absence of complications during the care period. The application of palliative nursing care for patients with chronic stroke and total dependence can enhance comfort, prevent complications, and support the quality of life for both the patient and their family through a holistic, patient-centered approach.
